Engaging Consumers for Improved Cost, Quality and Outcomes ...

Following closely on the heels of the mHealth Summit, the Bipartisan Policy Center (BPC) recently released the research report, ?Improving Quality and Reducing Risk in Health Care:? Engaging Consumers Using Electronic Tools?.? Arriving in the shadows of the fiscal cliff discussion, the report notes that healthcare spending currently places a considerable burden on our economy.? And with a focus on cost reduction, implementing the right measures to improve quality and individual health remain tantamount as well.

Originally identified in the 2001 report, ?Crossing the Quality Chasm: A New Health System for the 21st Century?, the Institute of Medicine (IOM) promoted patient-centeredness as one of six key aims for the nation?s health care system, thereby making it well-established and widely embraced today.? And building on the January 2012 report, Transforming Health Care: The Role of Health IT, the BPC identified six attributes that emphasize patient engagement?s role in supporting high performance organizations:

1. Organization-wide focus on the needs of the patient

2.? Strong organizational and clinical leadership

  • Bring the patient?s perspective to the design, delivery, and management of care.
  • Patients are involved in governance and advisory boards to provide input.
  • Create organizational culture that focuses on the needs of the patient and encourages continuous learning and improvement

3.? Access to information to support efficient, coordinated care

  • Successfully coordinate care across providers, settings, conditions, and time
  • Care teams and patients have access to an individual patient?s records across settings in which care and services are delivered.
  • Patient preferences and status are included in the record to inform clinical decision-making
  • Reminders and alerts for both clinicians and patients help eliminate medical errors as well as gaps and duplications in care
  • Privacy and security are carefully managed

4.? Timely access to care

  • High-performing organizations provide multiple avenues for patients to receive timely care.
  • Care teams are available when needed, whether by phone, by mail, online, or in person, including nights and weekends.

5.? Emphasis on prevention, wellness, and healthy behaviors

  • Recognize that providing educational resources and self-management tools to promote prevention and wellness, as well as management of chronic conditions can lead to better health outcomes

6.? Accountability, alignment of incentives and payment reform

  • High-performing organizations continuously measure and take actions to continually improve performance on the cost and quality of care, including patient experience of care and health outcomes.
  • Clinical, administrative, and patient-generated data inform goal-setting, the identification of areas needing improvement, and the effectiveness of interventions.
  • Financial incentives are aligned with better outcomes in cost, quality, and patient experience.

In efforts to support cost and quality goals, there have been a number of new models being tested by the Centers for Medicare and Medicaid Innovation (CMSI), including the Medicare Shared Savings, Advanced Payment and Pioneer ACO models, as well as the, Comprehensive Primary Care Initiative Demonstration.? Together with the Partnership for Patients:? Community Based Transition Care program, these arrangements are helping demonstrate that patient engagement, activation and communication are all associated with lower cost and better outcomes.

Despite the forward movement, significant challenges continue to exist for patient-centered care and patient engagement.? Such challenges include the need to expand patient engagement in medical education, post-graduate training and continuing medical education.? Making a fundamental shift in practice and reimbursement will not only improve physician-patient interaction, but also help to align participant goals with health, wellness and prevention.? And as Susannah Fox, Associate Director, Digital Strategy, Pew Internet & American Life Project, outlines below ? organizational culture should embrace the ?e? in e-Patient, since engagement is an instrumental step towards achieving the IOM goals.

Crop Insurance Juicy Target In Fiscal Cliff Deal

WASHINGTON ? Rural lawmakers worry that $9 billion in annual federal crop insurance subsidies are an easy target for spending cuts in a "fiscal cliff" deal so they're shopping around for a late compromise on a farm bill to protect them.

The chairmen of the House and Senate Agriculture committees already were set to make a sizable contribution to deficit reduction through a new farm bill. There were $2.3 billion in annual cuts to other farm programs and food stamps in a Senate-passed version and $3.5 billion in annual cuts in a measure awaiting House action.

But they hit an impasse this past week while trying to merge the bills into a single package. It's the same problem confronting President Barack Obama and House Speaker John Boehner, R-Ohio, who so far are unable to come up with an alternative to the broad wave of automatic tax increases and spending cuts that begin to hit in the new year.

The farm-state leaders hope that if they can agree on a farm bill, the White House and Boehner might include it as a deficit-reducing component of a broader fiscal cliff deal.

While cutting food stamps and other farm programs, both the Senate and House agriculture bills preserve crop insurance subsidies that cover a big portion of farmers' losses due to bad weather. Those subsidies buoyed a strong agricultural economy even through last summer's scorching drought. Committee leaders from both parties say farmers consistently tell them crop insurance is the program they depend on most.

But both bills also create an insurance program that would give farmers additional protection against any kind of revenue losses, such as from a drop in commodity prices. The Senate bill's version would cost almost $3 billion annually, with the House bill a little under $1 billion.

The concern about the fate of crop insurance stems from previous efforts by the Obama White House to target the program for cuts. Obama proposed cutting the subsidies by $760 million a year in his budget proposal last February. Conservatives long have eyed the program as a pot of money that could be used for other things.

Without giving details or numbers, Obama administration officials have made it clear in fiscal cliff negotiations that they see farm programs like crop insurance as a source for savings.

"Reforming farm subsidies, very important to do, lots of room to do sensible reforms in that context. And they can raise substantial amounts of money," Treasury Secretary Tim Geithner said on CBS "Face the Nation."

Agriculture Secretary Tom Vilsack warned the congressional committees this past week that, if they don't strike a deal soon, the White House and Republicans working to avert the fiscal cliff may cut farm programs that lawmakers want to protect.

"Don't let somebody else craft the farm legislation who may not fully understand all of the implications," he said.

Sen. Debbie Stabenow, who leads the Senate Agriculture, Nutrition and Forestry Committee, agreed.

"It's one of the many reasons we need to get a five-year farm bill done so people who don't support crop insurance don't come back at it and try to cut it," said Stabenow, D-Mich.

Farmers buy crop insurance to protect against weather-related losses such as drought or flooding. Farmers pay a percentage of the premium cost and the federal government covers the rest. Companies selling the insurance also get direct subsidies from the government.

Both the House and Senate made room for the new insurance program by eliminating a different type of subsidy called direct payments, in which farmers collect money from the government whether they farm or not and regardless of prices or crop yields. Cutting that program would save almost $5 billion annually and contribute to the overall savings in the bill.

Supporters of crop insurance say fewer subsidies would mean fewer farmers would buy coverage.

"Crop insurance proved its value once again this year by helping keep the rural economy on track and helping farmers pick up the pieces after a crippling drought," said Tom Zacharias, president of National Crop Insurance Services, an industry trade group. "Farmers are telling lawmakers to `do no harm' to crop insurance."

Critics say farmers could survive with much less federal help.

Crop insurance isn't farm-state lawmakers' only concern in the fiscal cliff negotiations. Milk prices could double if expiring dairy support programs aren't renewed before Jan. 1. Stabenow said she's also concerned there will be less money for a farm bill next year.

Attending lots of holiday parties this season?
Here are some tips to prevent overeating this time of year:

Drink WaterThe holidays bring to mind parties and large, festive meals full of delicious food. It can be easy to overeat during the holiday season, but there are ways to avoid overindulging while still enjoying yourself. Changing certain eating habits can make a big difference in calorie consumption without dampening the holiday spirit.

Step 1

Have a plan to avoid overindulging in holiday goodies. Bring healthy, low-calorie snacks like fruit to work to avoid snacking on high-fat snacks that coworkers bring in. If you fill up on low-calorie foods, you are less likely to overeat or eat large portions of high-calorie foods. Before attending a party where you know food (and fatty food, especially) will be in abundance, eat some nutritious snacks.

Step 2

Drink a glass of water before a holiday party or meal; this will help you feel full and reduce overeating. Drinking water throughout the day will also help keep you feeling full, making it less likely you will reach for excess goodies. Keeping a water bottle nearby is a good reminder to keep drinking water. And while at the party, instead of drinking soda or eggnog, ask for a glass of water. Sip on it constantly throughout the party so your stomach is always slightly full. In addition, it will save you the extra calories since carbonated beverages like soda and holiday drinks like eggnog are full of empty calories.

Step 3

Choose moderate portions of only the foods you like best during a meal, and only eat dessert if it is one you really like. Do not worry about hurting anyone?s feelings if you do not eat her food; if you really feel bad about not eating something; place a small portion of it on your plate. Reach for a small sized plate for your potluck or buffet dinner. By filling up the smaller plate, you will feel as if you are eating much more than you actually are.

Don?t skip meals.? As tempting as it can be to skip a meal or two before the large Christmas meal, this is a bad idea. Waiting to eat will trigger the starving reflex in your body, causing you to want to overeat. If you eat healthy, regular meals throughout the day, you are much less likely to overeat at the Christmas meal. Snacks throughout the day, such as vegetables and healthy nuts, will also help prevent overeating at mealtime.

Step 6

Focus on the reason for the gathering. Start up conversations with loved ones or friends and take the time to be present and engaged. The holidays are all about sharing good times with others; taking the focus off the food can discourage eating when you are not really hungry anymore.
